Scenario:
An employee is late for work numerous times over the course of the last evaluation period.
Sample Employee Appraisal Comments:
(Issue) You have been late for work numerous times over the past few months. Specifically, you came in 15 minutes late on Monday. (How to Improve) Set your alarm at least 20 minutes earlier, so that you arrive on time. (Warning) I expect you not to come in late again.
